Eastern Health Consultations Online Questionnaire
In follow-up to Eastern Health’s Portugal Cove-St. Philip’s area community consultations regarding Primary Health Care services (click here to view a copy of the in-person session workbook), held March 6-8, 2019, Eastern Health is now conducting an online questionnaire to provide an additional opportunity for people to provide input.
The questionnaire is open to everyone, regardless of whether you attended the consultation or not. We encourage you to take this opportunity to provide your input. The questionnaire will close on April 5, 2019 at 4:00 p.m.
